The McLean Hospital Neuropsychiatry and Behavioral Neurology (NBN) consult service provides mutltidisciplinary evaluations for inpatients with complex, multi-faceted disorders. Patients with psychiatric syndromes often also have neurologic disease or an unidentified dysfunction of the central nervous system. Similarly, patients with neurologic disorders often fall victim to devastating psychiatric symptoms. These patients therefore can greatly benefit from combined neurologic and psychiatric care.

Funded by the Sidney R. Baer Foundation, the NBN team consists of psychiatrists, neurologists. psychologists, and neuroscientists. As a group we evaluate patients and work together to generate comprehensive formulations and treatment plans. This multidisciplinary approach is time-efficient and cost-effective, giving the patient and the patient's treatment team rapid expert opinion and recommended courses of action. The NBN service also serves as an ideal forum for doctors-in-training allowing them to simultaneously become educated across fields of mind/brain medicine.

The members of the NBN team see the mind and brain as being inseparable; and with our expanding medical knowledge, the boundaries between psychiatry and neurology are becoming less and less distinct. The NBN has achieved a synergistic coalescence of the two disciplines, which empowers each field while providing cutting-edge training in mind/brain medicine and improving care for complicated patients.

Treatment Approach

The McLean NBN consult service meets twice weekly, in two-hour sessions, to examine and discuss patients. During each session, patients are presented by residents and fellows. The scope of medical, neurological, and psychiatric histories are explored, detailed mental status and neurological exams are considered, and a formulation is proffered. Then each patient is seen by the team allowing the members to observe and interact with the patient and apply their expertise. One of the staff clinicians also conducts a brief interview and performs a focused neurological exam. A comprehensive neurological write-up is then completed and made a part of the patient's medical record.

Programs and Services

Laboratory, imaging (McLean houses its own MRI), EEG, and comprehensive neuropsychological testing are brought to bear adding to the sophistication in the synthesis of the case. The team has an extensive network of services that includes state-of-the-art functional imaging, a sleep lab, quantitative electroencephalograpgy (qEEG), and an array of experts in diverse psychotherapeutic approaches. Each of the members of the core team are appointees of Harvard Medical School, thereby bringing the resources of the Harvard Community to the individual patient.
